Our doctors are accepting new patients. If you are being referred to us by a doctor, we will need your referring physician to fax over a prescription stating exactly why he/she is referring you here; whether it is for an evaluation or a specific procedure or both.

If you are being referred for a specific procedure, the prescription should include the name of the procedure that he/she would like you to have and the specific location. We will also need your last 3 office notes from the referring doctor, any diagnostic reports and your demographic information. We require all information prior to your appointment being made.

If you are a self referral and do not have a doctor referring you to us, please bring any diagnostic films or reports or office notes from any doctor pertaining to your problem to your appointment so that the doctor can get a history on you. 

Parish Pain Specialists will bill your insurance company for all charges for you.  Please bring your ID along with your insurance cards so we can scan those into the computer.  It is the patient’s responsibility to provide all necessary insurance information in order for us to obtain any necessary authorizations and submit our charges to your insurance company.  Parish Pain Specialists accepts most major insurances.  Please call the office to verify your benefits and inquire about any specific questions regarding your insurance coverage.
